Medical Emergency Inside a Locked Security Perimeter

A head of state's protective detail has locked down the venue. No vehicles in or out — including ambulances. A guest collapses with a cardiac event in the back of the hall, and the nearest paramedics are staged outside the hard line.

The situation

Fifteen minutes into the keynote, a guest near the back wall goes down. A doctor at table twelve starts CPR and shouts for an AED. Your ops lead is on the radio: the city ambulance is staged 300 metres out, the protective detail has the venue frozen, and their answer to 'open the gate' is 'negative, no movement.' The patient's clock is running.

First moves and what each one costs

• Deploy your inside medic team and AEDs to the patient now, and send your pre-briefed medical liaison to the detail's command post to negotiate a casualty extraction route — The medic team is on the patient in ninety seconds with an AED. The liaison — who the detail knows — agrees a protected corridor: the casualty moves out on a gurney through the service dock with a detail escort. Care never stopped.
• Keep pressing the detail to wave the ambulance through the gate — it's the fastest advanced care — Five minutes of radio argument buys nothing; the line holds. The doctor is doing CPR alone and no one has gone to the command post. You have spent the golden minutes negotiating instead of treating.
• Have staff carry the patient toward the perimeter gate to meet the ambulance — Moving a cardiac arrest patient stops effective CPR, and an unannounced group approaching the line is treated as a threat — the detail freezes the gate harder. You are now a security incident on top of a medical one.

Preparation notes for this situation

• Agree the medical plan with the protective detail before the event: who treats inside, how a casualty gets out, and who can open the line
